Due Date in IVF Pregnancies: How to Calculate

In assisted reproduction, the exact embryo transfer date is known, making the EDD calculation more precise than LMP-based estimation.

Day-3 vs. Day-5 embryos

  • For a day-5 (blastocyst) embryo: add 266 days to the transfer date. This is equivalent to counting from an "adjusted LMP" of 17 days before transfer.
  • For a day-3 embryo: add 263 days to the transfer date (266 − 3 days of in-vitro development).
  • Use the EDD calculator by entering the "adjusted LMP" calculated by your fertility clinic, not the biological LMP.

Examples

Blastocyst (day 5)

Input
Transferência: 10 de março
Expected output
DPP estimada: 27 de novembro (266 dias após a transferência)

Adjusted LMP is equivalent to February 23 (17 days before transfer).

Naegele's Rule adds 280 days (40 weeks) to the last menstrual period date to estimate the due date. It assumes a 28-day cycle and ovulation on day 14. Longer or shorter cycles shift the actual time of ovulation, which is why ultrasound is more accurate for confirming or revising the individual EDD.

Frequently asked questions

Why does my clinic use a different LMP than mine?

In IVF, the "adjusted LMP" is calculated by going back 17 days from the blastocyst transfer date (day 5), to align with the standard 40-week gestational count.
